Objective: On the use of methylene blue in a critical adult patient with refractory septic shock, treated in the ICU, the goal is to review the latest scientific evidence on its theme. Methods: Analysis of studies originally published in English between January 1995 to June 2011, with reference to the database in the U. S. National Library of Medicine (PubMed). Outcomes in selected references were: use of vasoactive drugs and hemodynamic parameters as mean arterial pressure, mean pulmonary artery pressure, and pulmonary vascular resistance. Results and discussion: The most favorable results articles made using methylene blue. There were no major discrepancies between the majority of studies in relation to hemodynamic parameters that describe benefits on the use of methylene blue in the recovery of mean arterial pressure. Conclusion: This review confirms the benefits of breastfeeding in the therapeutic management of refractory septic shock, both in reducing rates of vasoactive drugs, the increase in mean arterial pressure. And early treatment is the main component responsible for proven favorable results.
