Eficácia da imunoterapia alérgeno específica para o tratamento da rinite alérgica: uma revisão sistemática

Objective: To determine through a systematic review the effectiveness of specific allergen immunotherapy for the treatment of allergic rhinitis. Method: To carry out the research, the most relevant studies were selected, studies originally published in the English language, during the years 2000 to 2020, using the MEDLINE (National Library of Medicine) databases and the Cochrane Library as a reference. In order to select the studies with the greatest scientific evidence, we contemplate only the controlled and randomized clinical trials (ECCR) identified by previous reviews, which had their scope updated and revised, in addition to systematic reviews with or without meta-analysis. Results: 40 studies were identified involving allergen immunotherapy specific to the treatment of allergic rhinitis, however, only 6 were part of the scope of this review by the ECCR, that is, those with level of evidence “A”. Of these, it was observed that the largest number of articles were published in 2003, totaling 3 publications, and the other 3 in different years from 2001 to 2015, and in addition each of the articles used a specific type of allergen to verify the effectiveness of immunotherapy in the treatment of allergic rhinitis, in the table, attached the purpose of the study, the type of allergen studied, the number of participants and the results are illustrated. Conclusion: Immunotherapy is a type of treatment indicated and its main objective is to decrease the degree of sensitization of certain substances and, thus, inhibit the reactions. The use of allergen-specific immunotherapy for RA has many benefits, decreasing the symptoms of the disease and improving the quality of life of patients. However, research related to the mechanisms of immunotherapy is still ongoing and is necessary to fully elucidate how this form of therapy has an effect on allergic diseases.

Objetivo: Determinar por meio de uma revisão sistematizada a eficácia da imunoterapia alérgeno específica para o tratamento da rinite alérgica. Método: Para a realização da pesquisa foram selecionados os estudos mais relevantes estudos publicados originalmente na língua inglesa, durante os anos de 2000 a 2020, tendo como referência as bases de dados MEDLINE (National Library of Medicine) e a Biblioteca Cochrane. Objetivando selecionar os estudos demaior evidência científica, contemplamos somente osensaios clínicos controlados e randomizados (ECCR) identificados por revisões anteriores, e que tiveram seu escopo atualizado e revisado, além das revisões sistemáticas com ou sem meta-análise. Resultados: Foram identificados 40 estudos envolvendo a imunoterapia alergéno específica ao tratamento de rinite alérgica, contudo, apenas 6 fizeram parte do escopo desta revisão por os ECCR, ou seja, os que apresentavam nível de evidência “A”. Desses, observou-se que a maior quantidade de artigos foram publicados no ano de 2003, totalizando 3 publicações, e os outros 3 em anos diferentes de 2001 até 2015, e além disso cada um dos artigos utilizou um tipo de alergéno específico para verificar a eficácia da imunoterapia no tratamento da rinite alérgica, no quadro , em anexo são ilustrados o objetivo do estudo, o tipo de alérgeno estudado, o número de participantes e os resultados. Conclusão: A imunoterapia é um tipo de tratamento indicado e seu principal objetivo é diminuir o grau de sensibilização de determinadas substâncias e, assim, inibir as reações. O uso da imunoterapiaalérgeno específica para RA apresenta muitos benefícios, diminuindo os sintomas da doença e melhorando a qualidade de vida dos pacientes. Entretanto, pesquisas relacionadas com os mecanismos da imunoterapia ainda estão em andamento e são necessárias na completa elucidação de como essa forma de terapia exerce seus efeitos em doenças alérgicas.
