Objective. Determine, through a systematic review, which is the best technique for connective tissue grafting to gain periodontal and peri-implant gingival thickness, reducing patient morbidity and presenting better aesthetic results for the patient. Methods. Relevant studies published in English and Portuguese were analyzed, since the beginning of the 20th century and the last decade of June 2021, having referenced the MEDLINE (National Library of Medicine) and the Cochrane Library databases. The objective was to select the studies with the greatest scientific evidence. The search strategy we use keywords. Connective tissue. Transplantation, Keratinized gingivel, conjunctive graft. The inclusion and exclusion criteria were applied freely and independently, taking into account the proposed theme. Results. Regardless of the surgical technique or the moment of its execution, the band of keratinized tissue in the periodontal or peri-implant region is decisive in the maintenance of tissues adjacent to the periodontium and the peri-implant region, the thicker the band of keratinized tissue, the smaller it will be the chance of biofilm accumulation and periodontal tissue inflammation. Conclusions. Each patient must be individually assessed, noting which type of bone defect is involved, which is the best donor area for the patient, so that we can personally select the best surgical technique, which should be performed in the intervention, seeking the greatest gain in the thickness of the connective graft, both in the periodontal region and in the peri-implant region.

Objetivo. Determinar por meio de uma revisa~o sistematizada qual a melhor técnica de Enxerto de Tecido conjuntivo para ganho de espessura gengival periodontal e peri-implantar, diminuindo a morbidade do paciente e apresentando melhor resultados estéticos para o paciente. Métodos. Foram analisados estudos relevantes publicados em língua inglesa e portuguesa, desde o inicio do século XX até junho de 2021, tendo como referências as bases de dados MEDLINE (National Library of Medicine) e a Biblioteca Cochrane. O objetivo foi selecionar os estudos de maior evidência científica e a estratégia de pesquisa embasou-se nas palavras chave. Connective tissue. Transplantation, Keratinized gingivel, graft conjuntive. O critério de inclusão e exclusão foram aplicados levando em consideração o tema proposto. Resultados. Independente da técnica cirúrgica ou do momento de execução da mesma, a faixa de tecido queratinizado na região periodontal ou peri-implantar é decisiva na manutenção dos tecidos adjacentes ao periodonto e a região peri-implantar, quanto mais espessa for a faixa de tecido queratinizado, menor será a chance de acúmulo de biofilme e inflamação dos tecidos periodontais. Conclusão. Cada Paciente deve ser avaliado de forma individualizada, observando qual o tipo de defeito ósseo está envolvido, qual a melhor área doadora do paciente para selecionarmos de forma personalizada a melhor técnica cirúrgica, que deverá ser realizada na intervenção, buscando o maior ganho na espessura do enxerto conjuntivo, tanto na região periodontal como na região peri-implantar.
