Introduction. Lyophilized hydrophilized collagen sponge basically consists of two components: fibrinogen and animal thrombin (bovine) that promotes a mechanical barrier for hemorrhage, also serving as a framework where platelets adhere to a normal coagulation process. Its role as hemostatic is well known, however there is evidence that it may also favor alveolar healing. Objectives. The aim of the present study was to evaluate whether alveolar postoperative repair is favored by the presence of the fibrin sponge and its influence on alveolar healing and maintenance of bone height. Methods. An experimental study of the type clinical trial was carried out, through the evaluation of eight patients who showed indication of removal of two homologues, in the upper or lower arch. In one of the alveoli after the exodontia, the fibrin sponge was inserted. The other group represented the control group. Postoperative symptomatology and epithelialization were evaluated comparatively seven days after surgery. The alveolar bone height was verified two months after the exodontia, by periapical radiographic examination. Results. At the significance level, p <0.05, it was observed that there was no difference in bone height between the right and left sides, and the postoperative pain symptomatology was similar. Wound healing was better in the test group (p = 0.014). Conclusion. It was not possible to affirm that lyophilized hydrolyzed collagen sponge minimizes bone resorption after the exodontia. However, it seems to aid in initial healing without, however, substantially favoring the postoperative period.

Introdução: Esponja de colágeno hidrofilizada liofilizada consiste basicamente em dois componentes: fibrinogênio e trombina de origem animal (bovino) que promove uma barreira mecânica para hemorragia servindo também como um arcabouço onde as plaquetas se aderem simulando um processo de coagulação normal. Seu papel como hemostático é bem conhecido, no entanto há evidências de que possa favorecer também a cicatrização alveolar. Objetivos: O objetivo do presente trabalho consistiu em avaliar se o reparo alvéolar pós-exodontia é favorecido pela presença da esponja de fibrina e a influência que ela tem sobre a cicatrização alveolar e manutenção da altura óssea. Métodos: Foi realizado um estudo experimental do tipo ensaio clinico, através da avaliação de oito pacientes que apresentavam indicação de remoção de dois homólogos, na arcada superior ou inferior. Em um dos alvéolos após a exodontia, foi inserida a esponja de fibrina. O outro, representou o grupo controle. Avaliou-se assim, comparativamente, a sintomatologia pós-operatória e epitelização sete dias após a cirurgia. A altura óssea alveolar foi verificada dois meses após as exodontias, por exame radiográfico periapical. Resultados: Ao nível de significância p<0,05, observou-se que não houve diferença para altura óssea entre os lados direito e esquerdo, sendo semelhante também a sintomatologia álgica pós-operatória. A cicatrização da ferida mostrou-se melhor no grupo teste (p=0,014). Conclusão: Não foi possível afirmar que a esponja de colágeno hidrolizada liofilizada minimiza a reabsorção óssea após a exodontia. No entanto, parece auxiliar a cicatrização inicial sem contudo favorecer substancialmente o pós-operatório.
