Exercícios livres após cirurgia conservadora de câncer de mama: repercussões na amplitude de movimento e na qualidade de vida

Abstract
Fundamentation. Breast cancer is the most prevalent type of cancer in the female population and the second most common type of cancer in the world. Among the many possible sequelae of surgical and / or conservative, physiotherapy becomes an important therapeutic strategy to improve the quality of life (QOL) and physical and functional disorders. Objective. To investigate the effect of physical therapy intervention after conservative surgery for breast cancer, with a specific protocol of free exercises with the ipsilateral shoulder surgery and to examine whether these affect the range of motion (ROM) and QOL. Methods. A woman involved in the study. Application of general history and physical examination, the Medical Outcomes Study 36-item Short-Form Health Survey (SF-36), assessments of goniometry of the shoulder and girth. Were performed 10 sessions of physiotherapy, five times a week, at home of the patient. The protocol was applied consisting of preestablished free exercises that simulated the physiological biomechanics of the shoulder, to the maximum ROM. Results. At the end of treatment responses were tested regarding the history and physical examination, the domains of the SF-36, to goniometry and perimetry using descriptive statistics, noting decreased pain, no improvement in reported QOL, as well as the increase of ADM shoulder. There was no difference in perimetric assessments. Conclusion. The increasing ROM of the shoulder ipsilateral to surgery was considerable, and for the QOL there was no difference between baseline and final.
Resumo
Fundamentação. O câncer de mama é o tipo de câncer mais prevalente na população feminina e o segundo tipo de câncer mais frequente no mundo. Dentre as muitas possíveis sequelas do tratamento cirúrgico e/ou conservador, a fisioterapia se torna uma importante estratégia terapêutica para a melhoria da qualidade de vida (QV) e das disfunções físico-funcionais. Objetivo. Verificar o efeito da intervenção fisioterapêutica, após cirurgia conservadora de câncer de mama, com um protocolo específico de exercícios livres com o ombro ipsilateral à cirurgia e analisar se estes influem na amplitude de movimento (ADM) e QV. Métodos. Uma mulher participou do estudo. Aplicação de anamnese e exame físico geral, questionário Medical Outcomes Study 36-item Short-Form Health Survey (SF-36), avaliações da goniometria do ombro e perimetria. Foram realizadas 10 sessões de fisioterapia, 5 vezes por semana, no domicílio da paciente. O protocolo aplicado foi composto de exercícios livres pré-estabelecidos que simulavam a biomecânica fisiológica do ombro, até a ADM máxima. Resultados. Ao término do tratamento foram avaliadas as respostas referentes à anamnese e exame físico geral, aos domínios do questionário SF-36, à goniometria e à perimetria através da estatística descritiva, observando diminuição da dor, nenhuma melhora relatada na QV, bem como aumento da ADM do ombro. Não houve diferença perimétrica nas avaliações realizadas. Conclusão. O aumento da ADM do ombro ipsilateral à cirurgia foi considerável, e quanto à QV não houve diferença entre a avaliação inicial e final.
