Objective. To identify the existence of an association between montelukast and neuropsychiatric disorders. Method. We analyzed studies published in Portuguese and English, from 2001 to March 2018, in the National Library of Medicine (Medline/PubMed) and the Scientific Electronic Library Online (SciELO) databases. Prospective and cohort studies were considered. The primary endpoint of interest was defined as suicidal conditions, including: consummated suicide, attempted, ideation and suicidal behavior, separately or combined. Secondary outcomes included other montelukast adverse events such as: hallucinations, psychosis, personality disorders, agitation, hostility, irritability, depression, anxiety, mood disorders, hyperactivity, cognitive impairment, sleep disorders, seizures, and tremors. Results. The study included 15 literature that met the inclusion criteria, but only six were discussed: two studies have pointed to the influence of montelukast on the occurrence of neuropsychiatric disorders, such as insomnia, somnolence, hallucination, aggressiveness, irritability, hyperactivity, and personality, depression, suicidal ideation and behavior, and suicide; and the other four studies reviewed showed no clear association between montelukast and neuropsychiatric disorders, especially suicide, with insufficient data to prove the existence of this link. But it has been suggested that should investigate patients regarding mood swings and suicidal ideation. Conclusion. This review suggests that there is an association, although not statistically significant, between montelukast and psychiatric disorders. And it has been suggested that doctors should investigate their patients regarding mood swings and suicidal ideation.
Resumo
Objetivo. Identificar a existência de uma associação entre o montelukast e distúrbios neuropsiquiátricos. Método. Foram analisados estudos publicados em língua portuguesa e inglesa, entre 2001 a março de 2018, tendo como referência as bases de dados National Library of Medicine (Medline/PubMed) e Scientific Eletronic Library On-line (SciELO). Foram contemplados estudos prospectivos e de coorte. O desfecho primário de interesse foi definido como condições suicidas, incluindo: suicídio consumado, tentativa de suicídio, ideação e comportamento suicida, separadamente ou como resultado combinado. Os desfechos secundários incluíram outros eventos adversos do montelukast, tais como: alucinações, psicose, transtornos de personalidade, agitação, hostilidade, irritabilidade, depressão, ansiedade, transtornos de humor, hiperatividade, comprometimento cognitivo, distúrbios do sono, convulsões e tremores. Resultados. Fizeram parte do estudo 15 literaturas que preencheram os critérios de inclusão, mas somente seis foram discutidas: dois estudos apontaram a influência do montelukast para a ocorrência de distúrbios neuropsiquiátricos, tais como: insônia, sonolência, alucinação, agressividade, irritabilidade, hiperatividade e transtorno de personalidade, depressão, comportamento e ideação suicida, e suicídio; e os outros quatro estudos revisados não mostraram associação clara entre o montelukast e os distúrbios neuropsiquiátricos, especialmente o suicídio, com dados insuficientes para provar a existência desta ligação. Mas sugeriu-se que devem-se investigar os pacientes quanto a mudanças de humor e ideação suicida. Conclusão. Esta revisão sugere existir uma associação, embora não estatisticamente significativa, entre o montelukast e distúrbios psiquiátricos. E sugeriu-se que os médicos devem investigar seus pacientes quanto a mudanças de humor e ideação suicida.
