Avaliação radiográfica do risco de fratura radicular em dentes com núcleos metálicos fundidos

Abstract
Introduction. The cast posts are widely used retainers and depend on some ideal characteristics to alleviate the possible risks of fracture of the teeth restored by this technique when submitted to forces of occlusion, chewing, trauma and parafunction. Objectives. To analyze the characteristics of the cast posts by digital periapical radiographic analysis and to evaluate the risk of different types of fractures (longitudinal, oblique and transverse). Methods. The characteristics of intraradicular cast posts length, cast posts diameter, cast posts intraosseous insertion, root length, radicular diameter, root intraosseous insertion, were evaluated, measured and sorted in 1037 radiographs with cast posts and recorded in your own table.From the observed characteristics, the probability of occurring fractures of the longitudinal, transverse and oblique type was calculated by the chisquare test. Results. More than 90% of the cast posts were unsatisfactory in relation to the ideal parameters: the most common fractures were longitudinal (43%) and oblique (42%); no statistical significance was found between the cast posts characteristics and the type of fracture. Conclusion. The vast majority of the analyzed cast posts were unsatisfactory regarding root length, intraosseous length and diameter. Longitudinal and oblique root fractures were the most commonly observed in fractured teeth. It was not possible to establish the risk of root fracture only with the radiographic data.
Resumo
Introdução. Os núcleos metálicos fundidos (NMF) são retentores vastamente usados, e dependem de algumas características ideias para amenizar os possíveis riscos de fratura dos dentes restaurados por essa técnica, quando submetidos às forças de oclusão, mastigação, traumas e parafunção. Objetivos. Analisar, por meio de radiografias periapicais digitais, as características dos NMF e avaliar o risco de ocorrer os diferentes tipos de fraturas (longitudinal, transversal e obliqua). Métodos. Foram avaliadas, mensuradas e classificadas 1037 radiografias com NMF, as características de comprimento intrarradicular do NMF; diâmetro do NMF; inserção intraóssea do NMF; comprimento radicular; diâmetro radicular; inserção intraóssea radicular, anotadas em tabela própria. A partir das características observadas, foi calculada a probabilidade de ocorrer fraturas radiculares do tipo longitudinal, transversal e oblíqua, pelo teste Qui-quadrado. Resultados. Mais de 90% dos NMF se apresentaram insatisfatórios em relação aos parâmetros ideais; as fraturas mais comuns foram as longitudinais (43%) e oblíquas (42%); não foi encontrada significância estatística entre as características dos NMF e o tipo de fratura . Conclusão. A grande maioria dos NMF’s analisados se mostrou insatisfatórios quanto ao comprimento radicular, comprimento intraósseo e diâmetro. As fraturas radiculares longitudinais e oblíquas foram as mais comumente observadas em dentes fraturados. Somente com os dados radiográficos não foi possível estabelecer o risco de ocorrer fratura radicular.
