Imunoterapia sublingual no controle e tratamento da dermatite atópica: uma revisão sistemática

Objective. Determine through a systematic review, the use of sublingual immunotherapy in the control and treatment of atopic dermatitis (AD). Method. Previous studies originally published in the English language were analyzed, between the years 2007 and 2014, using the PubMed databases as a reference. Controlled clinical trials were considered, in addition to multicenter and observational studies in patients aged between 3 and 60 years old. Result. The analyze studies involved 306 patients aged between 3 and 60 years, with a 60% predominance for females. Most of the patients who underwent sublingual immunotherapy with standardized mite extract, presented favorable results compared to those who used only pharmacological treatment, with some studies finding values of up to 46% in the reduction of symptoms, which in one of the studies rated it 0 when there is no symptoms, 1 when there are mild symptoms, 2 when there are moderate symptoms and 3 when there are severe symptoms. All works used the SCORAD (Scoring Atopic Test) for better monitoring and classification. Conclusion. This review confirms the benefits of SLIT in the treatment of atopic dermatitis, reducing symptoms through the evaluated tool, which is SCORAD. Further studies are still needed, but it is possible to conclude that SLIT, when well used in those atopic patients, significantly improves the symptoms of AD and the complications related to this pathology.

Determinar por meio de uma revisão sistematizada o uso da imunoterapia sublingual no controle e tratamento da dermatite atópica (DA). Método. Foram analisados os estudos publicados originalmente na língua inglesa, entre os anos de 2007 e 2014, tendo como referência as bases de dados PubMed. Foram considerados os ensaios clínicos controlados, além de estudos multicêntricos e observacionais, em pacientes na faixa etária entre 3 e 60 anos. Resultados. Os estudos analisados envolveram 306 pacientes com idade variando entre 3 e 60 anos, havendo um predomínio de 60% pelo sexo feminino. A maioria dos pacientes que realizou imunoterapia sublingual com extrato de ácaro padronizado, apresentou resultados favoráveis comparado àqueles que utilizaram apenas tratamento farmacológico, tendo alguns estudos encontrado valores de até 46% na redução de sintomas, que em um dos trabalhos classificou em 0 quando não há sintomas, 1 quando há sintomas leves, 2 quando há sintomas moderados e 3 quando há sintomas graves. Todos os trabalhos utilizaram o SCORAD (Scoring Atopic Test) para melhor seguimento e classificação. Conclusão. Esta revisão confirma os benefícios da SLIT no tratamento da dermatite atópica, reduzindo os sintomas por meio da ferramenta avaliada, que é o SCORAD. Mais estudos ainda são necessários, mas é possível concluir que a SLIT quando bem empregada naqueles pacientes atópicos, melhora significativamente os sintomas da DA e as complicações relacionadas a essa patologia.
