Imunoterapia sublingual no tratamento da asma: revisão sistemática

Sublingual immunotherapy (SLIT) has been evaluated as a viable alternative in the treatment of asthma. The aim of this study was to describe the clinical and immunological effects of SLIT and its effectiveness in treating patients with mild to moderate asthma through a systematic review. Originally published studies were searched in English and Spanish, from October 1998 to March 2015, with reference to the MEDLINE database (National Library of Medicine). They considered only controlled and randomized clinical trials (ECCR) in the treatment of asthma. The analyzed studies involved 1323 patients aged between 5 and 50 years. The outcomes of interest for the search were: remission of seizures, reducing signs and symptoms, effectiveness of SILT and reduced medication. They were part of the scope of this review 6 ECCR that met the selection criteria. According to the study most of the ECCR showed favorable results SLIT compared to the usual care as treatment with corticosteroids base, some studies have found values up to 80.5% remission of asthma attacks. All ECCR efficacy and safety in the treatment with SLIT. It was concluded that treatment of immunotherapy is effective and safe for health, showing positive influence of SÇIT in the treatment of mild and moderate asthma, and reduce persistent asthma exacerbations.

A imunoterapia sublingual (SLIT) tem sido avaliada como uma alternativa viável no tratamento da asma. O objetivo deste trabalho foi descrever os efeitos clínicos e imunológicos da SLIT e sua eficácia no tratamento de pacientes com asma leve e moderada através de uma revisão sistemática. Foram pesquisados os estudos publicados originalmente na língua inglesa e espanhola, entre outubro de 1998 a março de 2015, tendo como referência as bases de dados MEDLINE (National Library of Medicina). Consideraram-se apenas os ensaios clínicos controlados e randomizados (ECCR) no tratamento da asma. Os estudos analisados envolveram 1323 pacientes com idade variando entre 5 e 50 anos. Os desfechos clínicos de interesse para a busca foram: remissão de crises, redução dos sinais e sintomas, eficácia da SILT e redução do uso de medicação. Fizeram parte do escopo desta revisão 6 ECCR, que preencheram os critérios de seleção. De acordo com o estudo a maioria dos ECCR apresentou resultados favoráveis a SILT quando comparado aos cuidados usuais como o tratamento à base de corticoides, tendo alguns estudos encontrados valores de até 80,5% de remissão das crises asmáticas. Todos os ECCR apresentaram eficácia e segurança no tratamento com a SILT. Concluiu-se que tratamento de imunoterapia é efetivo e seguro para a saúde, demonstrando influência positiva da SLIT no tratamento de asma leve e moderada, além de reduzir exacerbações de asma persistente.
