Efetividade da terapia antifúngica na aspergilose broncopulmonar alérgica: uma revisão sistemática

Abstract
Objective: To determine, through a systematic review, the effectiveness of antifungal therapy in patients diagnosed with Allergic Bronchopulmonary Aspergillosis. Method: We carried out a systematic review where the most relevant studies published were analyzed, without language restrictions, between 2013 and 2023, using the MEDLINE/PubMed (National Library of Medicine) databases as a reference. In order to select studies with the greatest scientific evidence, we only considered randomized controlled clinical trials (RCTs). The search strategy used the following combinations of keywords/descriptors, using the Medical Subject Heading tool: ("allergic bronchopulmonary aspergillosis" OR "allergic bronchopulmonary aspergilloses") AND (antifungals OR fungicides). To identify the study designs, the following terms were used: randomized controlled trial. Results: The scope of this review included 6 RCTs, which met the selection criteria. The studies analyzed involved 590 patients diagnosed with ABPA. Most RCTs showed favorable results for the use of oral or inhaled antifungals in the initial acute phase or relapse of ABPA, if used in combination with standard corticosteroid therapy; allowing dose reduction with fewer side effects. Most RCTs did not show superiority between the use of antifungals as monotherapy and conventional therapy. Conclusion: This review confirms the benefits of antifungal therapy in the therapeutic approach to patients diagnosed with ABPA, reducing their rates of annual exacerbations, in addition to contributing to improved lung function and reduced serum levels of total IgE, the most important biomarker in therapeutic failure. The most studied combination is itraconazole and prednisolone, demonstrating that antifungal agents act by decreasing the fungal load that reduces inflammatory activity and act as steroid-sparing agents. Antifungal treatment (itraconazole) is generally recommended for patients with ABPA who are steroid dependent, have frequent relapses, and where the benefits of treatment outweigh the risks. Other newer antifungals such as voriconazole show clinical improvement with a reduction in the need for oral glucocorticoids, improved asthma control and a decline in IgE levels; However, cost is a major current limitation. Nebulized lipid amphotericin B requires further study to determine efficacy; therefore, it is not currently recommended for the treatment of ABPA.

Objetivo: Determinar por meio de uma revisão sistemática a efetividade da terapia antifúngica nos pacientes com diagnóstico de Aspergilose Broncopulmonar Alérgica. Método: Realizamos uma revisão sistemática onde foram analisados os mais relevantes estudos publicados, sem restrição de idioma, entre 2013 e 2023, tendo como referência as bases de dados MEDLINE/PubMed (National Library of Medicine). Objetivando selecionar os estudos de maior evidência científica, contemplamos somente os ensaios clínicos controlados e randomizados (ECCR). A estratégia de busca utilizou as seguintes combinações de palavras-chave/descritores, por médio da ferramenta Medical Subject Heading: ("allergic bronchopulmonary aspergillosis" OR "allergic bronchopulmonary aspergilloses")AND (antifungals OR fungicides). Para identificar os delineamentos dos estudos, foram empregados os seguintes termos: randomized controlled trial. Resultados: Fizeram parte do escopo desta revisão 6 ECCR, que preencheram os critérios de seleção. Os estudos analisados envolveram 590 pacientes com diagnóstico de ABPA. A maioria dos ECCR apresentou resultados favoráveis para o uso de antifúngicos por via oral ou inalatória na fase aguda inicial ou na recidiva da ABPA, se utilizada associada à terapia padrão com corticóides; permitindo diminuição da dose com menos efeitos colaterais. A maioria dos ECCR não apresentaram superioridade entre a uso de antifúngicos em monoterapia e a terapia convencional. Conclusão: Esta revisão confirma os benefícios da terapia antifúngica na abordagem terapêutica de pacientes com diagnóstico de ABPA, reduzindo suas taxas de exacerbações anuais e diminuição dos níveis séricos de IgE total, biomarcador mais importante na falha terapêutica. A combinação mais estudada é o itraconazol e a prednisolona, demonstrando que os agentes antifúngicos atuam diminuindo a carga fúngica que reduz a atividade inflamatória e atuam como agentes poupadores de esteróides. O tratamento com antifúngicos (itraconazol) é geralmente recomendado para pacientes com ABPA que são dependentes de esteróides, têm recaídas frequentes e onde os benefícios do tratamento superam os riscos. Outros antifúngicos mais recentes como voriconazol apresentam melhora clínica com redução na necessidade de glicocorticóides orais, melhora no controle da asma e declínio nos níveis de IgE; porém o custo é uma grande limitação atual. A anfotericina B lipídica nebulizada requer mais estudos para determinar a eficácia; portanto, no momento, não é recomendada para o tratamento da ABPA.
